BRIEF
The Challenge
Catalunya's public health system faces growing pressure to transition toward integrated, patient-centred care that follows the person receiving care across settings and professionals. Digital tools are central to enabling this transition, but their development and implementation in a complex, multi-institutional system like SISCAT requires deep understanding of clinical workflows, professional needs, and organisational realities that cannot be captured through top-down planning alone.
The aims of the project were to:
Map the key healthcare processes where digital tools can add most value
Co-create digital assets with the professionals and patients who will use them
Develop a realistic, evidence-based roadmap to guide the implementation of these tools across the Catalan public health network over the coming years
FOCUS
The Approach
The project was structured into five phases, combining research and expertise with extensive participatory co-creation:
Systematic review of the scientific literature, combined with interviews with national and international experts in integrated care, digital health implementation, and healthcare transformation
SWOT analysis of the current situation in Catalunya, highlighting barriers and opportunities.
Mapping of key healthcare processes (type 2 diabetes, Duchenne muscular dystrophy, major depressive disorder) using BPMN methodology.
In-person and online co-creation workshops with healthcare professionals, persons receiving care, caregivers, and strategic leaders from across Catalunya, to co-design digital assets that address unmet needs in the current system.
Development of a realistic, phased implementation roadmap to guide the rollout of the digital assets across SISCAT.
CONCLUSIONS
The Envisioned Result
Extensive literature review on integrated care and digital health implementation, highlighting best practices, common obstacles, and real-world implementation examples.
Expert consultation report synthesising insights from national and international interviews, complemented by a SWOT analysis of the current digital health landscape in Catalunya.
A catalogue of digital assets co-designed with healthcare professionals and persons receiving care, covering functions ranging from care coordination and appointment management to clinical documentation support. This included the first mock-up of Catalunya's future Shared Electronic Health Record tool (“Història Clínica Compartida”)!
Process maps (BPMN) for key clinical pathways, integrating the digital assets into existing healthcare workflows.
A detailed implementation roadmap guiding the phased rollout of the digital assets across the SISCAT network over the short, medium, and long term.
A replicable co-creation methodology for future digital health transformation initiatives in Catalunya.

Our Role
Our role in the project consists of conducting the following tasks:
Systematic review of the scientific literature on digital health implementation, integrated care models, and healthcare transformation frameworks
Interviews with national and international experts to complement and validate the evidence base
Facilitation of the validation workshops with healthcare professionals, focused on collecting experiential knowledge to enrich the implementation roadmap
Synthesis and integration of all co-creation outputs, expert inputs, and literature findings into the final roadmap and process documentation.
